ALTERNATIVES PROGRAM

Helping you stay in your home

Our Alternatives program is geared towards adults who are low income and need assistance with activities of daily living. This program is one of our many programs that are meant to help older adults live at home.

Qualifications for program: 

  • Must be 18 years of age or older

  • Must meet program financial eligibility

  • Small fee may be required

  • Must have a need for assistance with care, as determined by professional risk assessment

  • Services received though local home care providers

Services Provided: 

  • Adult companion services

  • Case management

  • Chore services

  • Emergency response systems

  • Fiscal management services

  • Home delivered supplemental meals

  • Homemaker services

  • Medication reminder systems

  • Non-Medical transportation

  • Personal attendant program training

  • Personal attendant services

  • Personal budget assistance

  • Personal care assistance

  • Respite care assistance

  • Respite care services (may be provided in home or long-term care setting)

  • Specialized medical equipment
